Change of Name/Address

To ensure you are included in future newsletter mailings and surveys for the study, please let us know if you recently moved (or will move soon), or recently changed your name. Name and address changes should be sent to the study office at the address below. Please include your previous address (or name) along with your date of birth so we can be sure to update your information correctly.

Study Participant Death

Study participant deaths can be reported to the study by calling the study office at 612-625-1151 or by writing us at the address listed below. Please include the study participant's full name, date of birth, date of death, and state of residence at the time of death, along with your name and telephone number in case we need additional information.
